Assessment for depression and anxiety disorders

Key points

  • ⚠️ If you are having thoughts of harming yourself, call 115 or go to the nearest medical facility now. Do not wait for an appointment.
  • Insomnia and depression run both ways: insomnia raises the later risk of depression, and depression very often shows itself first as insomnia.
  • Insomnia remaining after mood has improved is a strong predictor of relapse, so it must be treated in its own right rather than treated as a remnant that will clear by itself.
  • A scale is a screening and monitoring tool, not a diagnosis. Diagnosis requires a clinical history.

Who this is for

  • Insomnia alongside low mood, loss of interest, or worry that will not stop.
  • Sadness, emptiness or hopelessness lasting more than two weeks.
  • Excessive worry, tension, an inability to relax.
  • You have taken the PHQ-9 or GAD-7 and scored in a range that needs assessment.
  • People close to you have noticed a clear change in your mood, sleep or eating.

These are treatable, and treated early they move faster

The biggest barrier in Vietnam for these two conditions is not a shortage of methods but the waiting. A great many people come after two or three years, having tried everything else. That interval is not neutral: it lets the illness set, costs jobs, breaks relationships, and in depression it carries real risk.

Sleep is where the two meet

In practice, insomnia is what finally brings most patients in, even when the larger problem is mood. That is understandable: not sleeping is a concrete symptom, it can be said out loud, and it carries no stigma. "I think I am depressed" is a far harder sentence.

So a good insomnia consultation always includes questions about mood and anxiety, and the reverse holds too. The two sides cannot be separated, and treating one while ignoring the other usually gives a half result.

RelationshipWhat is knownWhat it means for treatment
Insomnia → depressionInsomnia is an INDEPENDENT risk factor for later depressionTreating insomnia is a preventive measure, not only symptom relief
Depression → insomniaDisturbed sleep is a very common symptom of depression; early-morning waking is the classic patternLow mood with waking at 3 or 4am must be assessed for depression
Residual insomniaInsomnia left after mood has improved is a strong predictor of relapseThe sleep component must be treated in its own right, not assumed to clear on its own
Anxiety ↔ insomniaWorry about not sleeping is itself a mechanism maintaining insomniaThe cognitive part of CBT-I aims straight at this loop

About privacy

We do not publish patient testimonials, we do not quote comments, and we do not use anyone's story as marketing content. What you say in a consultation is health information, and it does not leave the consultation.

How it works

  1. Safety assessment before anything else

The consultation starts by asking directly about thoughts of self-harm. That question does not increase risk, the evidence points the other way, and the answer shapes every step afterwards. If there is acute risk, safety comes before any treatment plan.

  2. History and validated scales

PHQ-9 for depression and GAD-7 for anxiety, alongside a full history. A scale gives a numerical reference point so the next visit has something to compare against. That is its main value, not labelling.

  3. Ruling out physical causes

Thyroid disease, anaemia, vitamin B12 deficiency, neurological disease and certain medicines all produce a picture resembling depression or anxiety. Skipping this step leads to treating in the wrong direction for a long time.

  4. Establishing the relationship with sleep

Whether insomnia came first or low mood came first changes the order of treatment. Insomnia is also an independent risk factor for depression, and insomnia remaining after depression has been treated is one of the strongest predictors of relapse.

  5. Treatment plan and follow-up

Depending on severity: psychological therapy, medication, or both, with a specific follow-up schedule. Mild presentations are usually reviewed in 2 to 4 weeks rather than treated immediately.

Frequently asked questions

Does being assessed for depression mean starting medication straight away?
No. Mild presentations are usually reviewed in 2 to 4 weeks, and psychological therapy stands alongside medication in many situations. The decision depends on severity, other conditions and your own preference.
My PHQ-9 score is high. Does that mean I have depression?
A high score means you should be fully assessed, not that a diagnosis has been made. Many other conditions raise the PHQ-9, including severe insomnia on its own and thyroid disease.
Will my information be kept private?
Yes. Health information is not shared outside the scope of your care, and it is never used as marketing material.
